Firebrick and refractory panels crack with thermal cycling, and once a crack opens more than a joint's width, heat has a path toward the wall structure. Fireplace & Firebox Repair Questions

The commonly used threshold is around an eighth of an inch — roughly a joint's width. Wider than that, in a panel or in firebrick, means heat has a path toward the structure behind and the firebox should not be used until it is repaired. Hairline crazing from thermal cycling is normal and is not the same thing.

Many can be repaired, but only with the manufacturer's listed parts for that specific unit. A prefabricated fireplace is certified as an assembly; substituting firebrick or generic panels voids the listing and the insurance position that goes with it. Where parts are no longer available, replacement is the honest answer and we will say so.

Most often the smoke chamber, not the flue. A rough, unparged chamber with corbelled ledges disrupts the transition into the flue and pushes smoke back into the room. Parging it smooth is a common fix. The other usual causes are an undersized flue, an oversized opening, and a house too tight to supply makeup air.

If your throat damper is failing, generally yes. It seals at the top of the flue with a gasket rather than metal-on-metal at the throat, so it stops the year-round draught up an open flue much more effectively, and it keeps rain and animals out as a side effect.
